INSURANCE:

We will be glad to file your insurance claim if you belong to an insurance program that we contract with.  Please bring your insurance card if you choose to have us file your claim.

If you have any questions regarding your health plan, or problems with payment, please feel free to contact our patient representative at 704-864-2650.  Responsibility for all charges remains with the patient regardless of insurance coverage.

Medicare
We accept assignments and file your claim for you.  However, the patient is responsible for deductibles and any charges not covered by Medicare, including minor office procedures.
